Filming & Photography
The show will be recorded as a whole-group performance to be offered for purchase to families. Because this is a live group event, individual children cannot be blurred or edited out of the video footage; because of this, video consent is a strict condition of participation in the show. By submitting this form, you consent to your child appearing in this show footage and in official photography, which may be shared with parents and used for OBDA promotional purposes (including our website and social media). If you require your child to be excluded or blurred from public promotional photos only, please contact us directly after submitting this form so we can update our records.

Costumes
You agree to supply small items if requested to complement costumes (e.g., specific shoes, undergarments, or tights). More info will follow.

Assistance
You consent to OBDA DBS-checked helpers supervising and assisting your child, including with costume changes, and toilet use if needed.

Medical & Emergencies
You confirm any relevant medical info has been provided, and that your child is fit to perform. In an emergency, you consent to basic first aid, for emergency services to be contacted, and for OBDA staff to authorise necessary medical treatment if parents/guardians cannot be reached immediately.

Behaviour & Supervision
Your child will be supervised by OBDA staff and volunteers and is expected to follow instructions and behave appropriately throughout. Persistent, disruptive or unsafe behaviour may result in your child being removed from the performance and requiring immediate collection.

Drop-off & Collection
You agree to follow sign-in/sign-out procedures and ensure your child is dropped off and collected by an authorised adult.

Data Protection
OBDA will store and use your details in line with GDPR, only for show-related purposes.

Show Fee
The show fee (already added to your invoice in April) contributes to production, participation medal, costume, and rehearsal (July 5th) costs, many of which are costs paid in advance. If your child withdraws after the form has been submitted, the fee will not be refundable.

Cancellations Beyond Our Control In the unlikely event that the show must be cancelled or rescheduled due to circumstances beyond OBDA’s control (e.g., venue failure, severe weather), we will make every effort to arrange an alternative date, but show fees remain non-refundable due to advance production and costume overheads.
